1. Initial discharge

Once your hospital has declared you medically fit, you may still require support with moving back home.

Once you contact us, we can start gaining an understanding of your situation.

3. Care Assessment

We can provide you with an in-person, friendly care assessment. We will come to your house to meet you and your family.

4. Create a care plan

After our assessment, we create a care plan that best reflects your needs and goals – this is your plan to thrive.

5. Meet your carer

We use our first care visit as a chance for you to meet a member of our care team.

6. Ongoing review

We will review your care on an ongoing basis, working with you to adjust hours and care plans to better support you.

This depends on your individual situation. Many people who are deemed medically fit, still require help. That may be support with washing and dressing, eating and drinking, or general mobility. Whatever your goals, we can provide care that best reflects them – helping get to where you want, more quickly.

We train our carers in-house, the Walfinch way. They undergo comprehensive and specialist training so that they can care for people with a wide range of needs. We have very high standards regarding training – meaning our carers must pass a series of in-person examinations, training sessions and online learning before helping people in their own homes.

Both yourself, as the client, and your family can benefit from well-managed transitions. Support with transitions such as from hospital to home, or downsizing, can have a large emotional impact. Knowing you have a team of professionals dedicated to your health and wellness, helps take some of the stress away.
