Understanding transitional support
Change is difficult – and not always easy to embrace.
With transitional support, our Walfinch carers can help make life’s transitions more manageable. We all have our individual needs, but whatever your situation, we can find ways to help you thrive.
That’s why we pride ourselves on providing care that embraces the individual, always ensuring to take your personal situation, preferences, hobbies and goals into account.

How can Transitional Support help you?
In the past, we have supported people with:
- Moving back home after long periods in hospital
- Adapting your lifestyle to rapidly changing needs
- Down-sizing to a smaller home
- Moving from a residential care home, back to your own home
Care that changes with you
Our care isn’t one-size-fits-all. We’re all individuals, so why should your care options all be the same? Unlike the support often found in residential care homes, Walfinch home care can tailor your care to you – working 1-1 with you and your family, to help you get the most out of life.
At-home care can help you to:
- Enhance your quality of life
- Provide emotional support through challenging transitions
- Keep your family involved with your wellbeing
- Provide you with greater independence
- Support and streamline your recovery, if required

Transitional support you can trust
Good transitional support is well-managed, organised, and compassionate to your situation. Our carers can work with your current healthcare providers to ensure your transition is as smooth as possible.
